Item Checkout Request

Use one of the following forms to request items from RSA. If you are a resident doing an event for a residence hall please use the first form. Otherwise please use the second form.

**Any Request can be Denied if the form is not submitted in enough time **

** ALL Requests are due no later than 2 weeks in advance of planned events.**
